The flea-flicker is slightly more involved than just play-action. The quarterback-to-RB exchanges can be messed up quite frequently; a lot of fumbles, miscues, the like. So expecting a proper center to QB to RB back to QB exchange 4 times a game is risky.
Also, the flea-flicker causes problems for the quarterback, who can't watch the field the whole time, as he needs to watch for a pass back from the RB. That's why they use it more often for deep throws, since its easier for the QB to just look up and chuck it, rather than read real routes and the entire defense.
