Kenan Blackshear scored 22 points, Jarod Lucas added 15 and Nick Davidson 14 points for Nevada (19-5, 6-4 MWC), which has upset ranked opponents in back-to-back games for the first time in program history.
The Wolfpack were coming off a 77-63 win at No. 22 Utah State, its first true road win against a ranked opponent since 1981.
Jaedon LeDee scored 20 points, Darrion Trammell added 11 and Micah Parrish had nine points and 10 rebounds for San Diego State (18-6, 7-4), which had just re-entered the AP Top 25 rankings for the fourth time this season.
LeDee made two free throws and scored an inside basket for a 64-60 lead in overtime, but Nevada scored the next six points to move ahead 66-64.
Nevada scored the first six points of the game and didn't lead by more than seven points or trail until Jay Pal converted a three-point play to give the Aztecs a 53-52 lead with 3:48 left in the second half.
LeDee scored on a post move to extend the advantage to 55-53, and Trammell sank a jumper to make it 57-54 with 1:37 to go.
LeDee made 1-of-2 free throws for a 60-58 lead with 24 seconds remaining, but Davidson grabbed an offensive rebound on the other end and was fouled as he laid the ball in with 8.5 seconds left.
He missed the free throw, however, keeping the score tied at 60-60, and the Aztecs couldn't convert on the other end.
The Aztecs came in 30-for-65 from 3-point distance in its past three games (46.2 percent), but shot just 3-for-19 (15.8 percent) from deep against Nevada.
The Wolfpack took a 30-27 lead into the break.
