Spears was injured late in a Week 11 loss to the Minnesota Vikings. He rushed three times for zero yards and had two catches for eight yards.
Spears previously missed games in Week 7 against the Buffalo Bills and Week 8 versus the Detroit Lions with a hamstring injury.
In seven games this season for the Titans (3-8), Spears has 161 rushing yards and one touchdown on 43 carries and 17 receptions for 88 yards.
The 23-year-old was a third-round draft pick of Tennessee in 2023.
While Spears is active on Sunday, the Titans listed the following as inactive: running back Joshua Kelley, cornerback Gabe Jeudy-Lally, offensive lineman Leroy Watson and tight end David Martin-Robinson.
As for the Commanders (7-5), they listed quarterback Jeff Driskel, cornerback Marshon Lattimore, tackle Andrew Wylie, linebacker Dominique Hampton and defensive back Phidarian Mathis as inactive.
