Financial terms of the deal were not disclosed by the team. However, NFL Network reported it was worth $5 million.
"This is where I want to be," Karras told the team's website. "People here have been so great to me on and off the field, and the goal remains the same. To win a championship."
Karras, 32, has started all 50 games in which he has played for Cincinnati over the last three seasons.
A two-time Super Bowl champion with the New England Patriots, Karras will make his 100th career start on Sunday when the Bengals play their season opener against the host Cleveland Browns.
Karras has played in 143 career games with the Patriots, Miami Dolphins and Bengals since being selected by New England in the sixth round of the 2016 NFL Draft.
In March, Karras joined the executive committee of the NFL Players Association's board of player representatives as vice president.
