Realmuto departed Sunday's road game against the Los Angeles Dodgers in the fourth inning, one inning after he was hit by a pitch on the left wrist.
The Phillies later announced that Realmuto had a wrist contusion and would undergo further testing.
Realmuto was hit on the left hand by a 97-mph fastball from Dodgers right-hander Yoshinobu Yamamoto in the third inning but remained in the game. He was replaced by Rafael Marchan on defense one inning later.
Marchan had been on deck to bat for Realmuto in the top of the fourth.
Realmuto is batting .220 with two home runs and nine RBIs in 39 games this season. A three-time All-Star, the 35-year-old owns a .269 career average with 182 home runs and 686 RBIs in 1,412 games over 13 seasons with the Miami Marlins (2014-18) and Phillies.
