TSN said the deal is believed to be worth $92 million, or an average annual value of $11.5 million. It also is expected to carry a full no-trade clause.
The Maple Leafs selected Nylander with the eighth overall pick in the 2014 NHL Draft.
He's played in 558 career games with Toronto, accumulating 484 points (198 goals, 286 assists). His 54 points in 37 games this season are tied for fifth in the NHL and lead the Maple Leafs, as do his 33 assists.
Nylander, 27, would be the latest big signee for the Maple Leafs.
Last August, Auston Matthews signed a four-year, $43 million contract extension that takes effect with the 2024-25 season. Forwards John Tavares and Mitch Marner also carry cap hits of more than $10 million.
Nylander is playing on the final season of a six-year, $45 million contract signed in 2018.
